Types of gum disease

Gingivitis

It is an inflammation of the gingival margin (including the part adjacent to the tooth cervix) due to poor oral hygiene.

The reason:

Bacterial plaque and tartar accumulate near the necks of the teeth. The pathogenic bacteria they contain release toxins that trigger an inflammatory process in the periodontal tissues.

The symptoms:

  • Occasional bleeding and soreness of the gums while brushing the teeth
  • Redness or lividity of the gingival margin
  • Swelling of the gingival papillae
  • Bad breath

The solution: Ultrasonic teeth cleaning.

Gingivitis

Periodontitis

It is an inflammatory process in the tissues of the gingival attachment (gradually destroying it) and in the periodontal bone tissue. It causes tooth loosening, which, without timely treatment, can lead to tooth loss.

The reason:
Lack of treatment of gingivitis, which leads to further spread of inflammation. Incorrect traumatic bite.

The symptoms:

  • Root exposure
  • Accumulation of subgingival tartar
  • Formation of periodontal pockets (may be accompanied by purulence)
  • Loosening or displacement of teeth.

The solution: Ultrasonic cleaning + surgical treatment (curettage and flap surgery) + normalization of bite with GNM.

Periodontitis

Pericoronitis

Inflammation of the mucosa over an erupting tooth due to an infection.

The reason:

  • Pathologically dense bone tissue in the area of teething
  • Thick gingival biotype over the teething tooth
  • Denser cortical plate due to an anomaly in the development of the jaw.

The symptoms:

  • Swelling or redness of the gum
  • Pain (may extend to neighboring areas)
  • Inflammation of regional lymph nodes
  • Painful opening of the mouth.

The solution: Medication therapy + surgical treatment (excision of the hood or removal of the causal tooth).

Pericoronitis

Gum boil

Infectious inflammation in the periosteum — a connective tissue surrounding the tooth. A purulent pouch forms inside the gum, which enlarges and causes the soft tissue to swell.

The reason:

  • Lack of timely treatment of caries/pulpitis (with infection penetrating the pulp, developing, and causing purulent inflammation)
  • Poor-quality treatment of root canals (with infection developing in the unsealed areas of the canal and causing a purulent focus)
  • Exacerbation of periodontal disease (a fistula occurs due to the violation of the outflow of pus from the deep periodontal pocket).

The symptoms:

  • Nagging pain in the area of the affected tooth, intensifying when pressure applied
  • Swelling of the gum
  • Swelling on the cheek
  • Lymph-node hyperplasia
  • Temperature increase up to 38 degrees Celsius.

The solution: Anti-inflammatory therapy + root canal treatment + curettage (if indicated).

Gum boil

Gingival hyperplasia

Pathological overgrowth of periodontal tissues, which causes most of the tooth to close.

The reason:

  • Side effects of certain medical drugs
  • Hormonal disorders
  • Teeth crowding.

The symptoms:

  • Gum covering ⅔ of a tooth or more
  • Enlargement of the gingival papillae
  • Bleeding when brushing teeth or eating.

The solution: Gingivectomy (see below).

Gingival hyperplasia

Gingival recession

Abnormal loss of gingiva, leading to gradual bareness of the tooth root.

The reason:

  • Untimely treatment of periodontitis
  • Permanent trauma to the gum by the edge of the crown or brace
  • Incorrect bite.

The symptoms:

  • Bleeding, swelling of the gums
  • Gum flaking from the surface of the tooth
  • Hypersensitivity of the teeth to irritants.

The solution: Orthodontic treatment + gingivectomy (Periodontal Surgery).

The patient had been bothered by gum inflammation for many years. She was treated at various clinics, but the problem was not solved. It got to the point where her teeth began to come loose. She decided to go to Dr. Tetri. With his knowledge of neuromuscular dentistry, Dr. Tetri identified the cause of the gum problems, corrected it, and then placed the implants and immediately loaded them with fixed prosthesis. Computed tomography allows you to look at the picture from all sides and projections, with an accuracy of a fraction of a millimeter to detect altered areas and to make a reliable diagnosis.

MYOGRAPHY OF OROFACIAL MUSCLES

Gum disease is often caused by an abnormal bite or malfunction of the temporomandibular joint. In-depth diagnosis of the condition of the masticatory muscles at the neuromuscular level reveals “hidden” problems with bite or TMJ dysfunction.

Treatment options

GINGIVECTOMY

The surgical procedure to remove overgrown gum tissue is performed when treatment is ineffective.

GINGIVOPLASTY

The gingiva is peeled off during the surgery, a new gingival contour is corrected, and sutures are applied.

Gingivectomy
CURETTAGE

The mucosa is detached from the surface of the teeth, deposits are removed from the roots, an artificial bone is implanted (only if severe atrophy is present), and sutures are applied.

Curettage
MICROSCOPE

Multiple optical zoom allows for minimal invasiveness of gum surgery: neat incisions/sutures (fast healing), thorough removal of granulation or necrotic tissue, and excellent aesthetic results.

Microscope

Includes a set of procedures: removal of plaque and calculus with AirFlow — a course of antiseptic rinses and applications of anti-inflammatory gel on the gums.
